Overview
The Real-Time Scrubber for CMS-1500 helps healthcare providers prevent costly rework by identifying and fixing claim errors before submission.
It reviews your claim against 50+ validation areas — combining CCI edits, e Physician Fee Schedule rules, CPT® and HCPCS guidance, LCDs, and NCDs — to detect code conflicts, modifier misuse, and other top denial triggers.
All alerts include plain-language explanations so you can correct claims quickly and confidently.
The scrubber is available with Pro Fee and Complete Coder subscriptions.
Accessing the Scrubber
Go to the Coding Tools tab.
Select Real-Time Scrubber (CMS-1500).

Entering Claim Information
Step 1: Apply and Select Date of Service
If all procedures share the same service date, click Apply Date of Service to All Rows.
Then select the date using the calendar picker.

Step 2: Enter the First Procedure Line
Procedure Code
Type your CPT® or HCPCS code, or click the 🔍 Search icon to open the SmartSearch modal.
Use SmartSearch filters to look up codes by keyword, description, or set.

Click the desired code to insert it directly into your claim — no need to leave the page.
Units
Enter the number of units for the service.
Modifiers
After entering a CPT code, click the Modifier Crosswalk button that appears.
Review the list of valid modifiers for that procedure.
Select and insert the appropriate modifier with one click.
This ensures modifier accuracy and prevents common denial causes.

Diagnosis Codes
Enter the ICD-10 diagnosis codes associated with the procedure.
Step 3: Add Additional Procedure Lines
Repeat the same process for each additional procedure:
Enter procedure code (using SmartSearch if needed)
Enter units
Add modifiers using the Modifier Crosswalk
Enter diagnosis codes
Managing Claims with the Claims Library
You can now save, reuse, and manage claims directly within the scrubber.

Submitting and Reviewing Results
After all information is entered, click Submit.
The scrubber instantly reviews your claim across 50+ validation areas.
Results display on-screen with:

Each explanation is written in plain English — no complex coding jargon.
Saving Your Results
Click Download PDF to generate a detailed report of your scrubber results.
